Key?
Do the naturals for some Cs imply that the others should be sharp? So, should the key really be D major or are all the Cs supposed to be natural?

Yes. The naturals for some C's imply that the other shall be C#' . ie In Bar number 1, 5 and 11 = c natural.
Hmm. not a clear tonality, mayby scottish in inspiration?

IS with Csharp everywhere except where C NATURAL is specified.
Isn't that DMAJOR? (mixolydian)?
