It certainly looks like the grid from a Hohner Erica, but the buttons are a bit too fancy looking to be from a Hohner.
J Ts clip looks like a well worn Hohner 'Poker Finish'
The mop buttons are obviously replacements, probably the keyboard too.. It's a nice example of a stripped-down-then-tarted-up Hohner Double Ray or Erica, which are essentially the same box anyway.
I would like to have a go at "tarting up' a Double Ray/Erica myself, does anyone have any info on where I can source materials such as reeds, bellows 'mop buttons etc. or know of any websites that may be helpful?
its definitely a Honer Double Ray stripped back. pretty much exactly the same as one I was given by a relative, minus the woodworm. Sounds like a new set of reeds in there too. A lot drier then the normal reeds.
The first thing you need to do to soup up a Hohner is to fix the button travel - stop the buttons disappearing down the holes.
There are various ways of doing this and you'll find lots of info. on this and the other things you ask about on the forums at http://forum.melodeon.net/
Or you can get a replacement keyboard from Irish Dancemaster in Florida.
New Italian reeds will change the sound of the box - I like the Hohner sound and definitely wouldn't replace the original reeds unless they were shot. And if they were I think I'd look for old Hohner replacements.
Nor would I replace the bellows unless they were completely shot. You can prolong their life and improve the appearance by retaping them and possibly reinforcing them inside.
You may well want to dry out the tuning a little bit, or a lot, depending on your taste. This isn't really a DIY job - if you want to try, best to practise on a scrap accordion first because everyone will tell you it is very easy to ruin reeds until you know what you are doing.
If you are going to get the keyboard taken care of by IrishDancemaster, let Michael do the reeds also.
Even though my main box has become a Saltarelle, I love to go back to box he did for me a few years ago because the reeds are so nice after he redid them.
What box is this?

Re: What box is this?
In this clip it looks like a Hohner Erica or Black Dot stripped down to the wood and doubtless souped-up (not a clack to be heard):
